Introduction to Chronic Disease: Impact & Risk Factors
In this tutorial we will look at patterns of chronic disease in the United States. We see how rates of chronic disease can vary by age, sex, race, ethnicity or geographic location. We will also look at several chronic disease risk factors such as smoking, obesity and high blood pressure and see what patient navigators can do to improve patient health.
