Patient outcomes improve when patients receive emotional, administrative and educative support. Now that healthcare organizations realize its importance, they are making efforts to improve patient-centered care. Patient-centered criteria is now incorporated into licensing and accrediting programs and organizations are also using a team-based approach to training physicians, nurses, and office administrators on patient-centered practices.

How do navigators address health care system barriers? By working with the health care team.
Some barriers to care come from within the health care system. For example, if a patient refuses care due to language barriers, cultural beliefs or mistrust of the health care system, the navigator must help members of the healthcare team understand these patient issues. Navigators may also need to work to resolve communication problems, coordinate care, or scheduling conflicts within the healthcare system.
